The application for the connection permit shall be in writing on a form provided by the Town, which shall require at least the following information, together with any plans, specifications, or other information considered pertinent in the judgement of the superintendent:

A. Name and address of applicant;

B. Name and address of owner of the premises where the connection is to be made, or where the drain or line is to be laid;

C. Location of the proposed connection, drain or sewer pipes;

D. Statement as to the type, materials and method of connection and the type of materials to be discharged into the sewer. (Ord. 500 §1, 2004)
